Case Management
A multidisciplinary team that coordinates complex care, from hospital admissions to required paperwork to community resources.
How we can help.
Our case management team coordinates the full spectrum of care needs, from direct hospital admissions to required evaluations and forms. The team includes physicians, APRNs, LPNs, and licensed social workers.
From the moment a complex care need arises, our case managers work across facilities, systems, and agencies so that care stays coordinated and nothing falls through the cracks.
Direct hospital admissions
Our physicians can directly admit patients to partner hospitals and skilled nursing facilities, bypassing the ER when clinically appropriate. That reduces stress and keeps care with a physician who already knows you.
PASRR evaluations
Pre-Admission Screening and Resident Review (PASRR) evaluations are federally required before skilled nursing facility admission for patients with mental illness or intellectual disabilities. Our team completes Level I screenings and coordinates Level II evaluations.
3008 form completion
Florida Medicaid requires the AHCA 3008 Physician Certification Form for long-term care placement. Our providers complete these forms accurately and promptly to support timely placements.
Care transitions coordination
Coordination across hospital, skilled nursing, assisted living, rehab, home care, and clinic settings, so nothing falls through the cracks when patients move between levels of care.
Social services navigation
Licensed social workers identify and address housing instability, food insecurity, transportation barriers, and financial hardship, and connect patients with community resources and benefits programs.
Family and caregiver coordination
Families stay informed and involved in care decisions. Case managers communicate with family members, help navigate care options, and support the patient’s expressed goals and wishes.

The case management process.
Referral and initial assessment
A need is identified by a physician, facility, family member, or the patient. A case manager assesses medical, functional, psychological, and social needs.
Care plan development
Working with the patient’s physician and team, the case manager builds an individualized plan for immediate needs and long-term goals.
Documentation and compliance
PASRR forms, 3008 certifications, prior authorizations, and other required documents are completed accurately and submitted promptly.
Facility and service coordination
The case manager coordinates with receiving facilities, home health agencies, equipment suppliers, and community services.
Ongoing monitoring and adjustment
After placement or transition, the case manager keeps monitoring the patient, adjusts the plan as needs change, and steps in early to prevent avoidable crises.
Key forms and regulatory processes
Our case management team is experienced with Florida’s long-term care requirements, so documentation stays timely and compliant for every patient.
Florida 3008 physician certification
Required by Florida Medicaid for long-term care placement. It certifies that a patient needs the level of care provided in a nursing facility.
PASRR Level I and II
Pre-Admission Screening and Resident Review is federally required for Medicaid-funded skilled nursing admissions involving mental illness or intellectual or developmental disabilities.
Prior authorization support
Clinical documentation and physician attestation for insurance prior authorizations, including skilled nursing admissions, home health, equipment, specialist referrals, and advanced diagnostics.
