Our Services

Pharmacy Support

Your medications, managed by our on-staff clinical pharmacist, an expert on your care team.

About This Service

How we can help.

Our on-staff clinical pharmacist is part of your care team, reviewing medications, identifying risks, counseling patients, and partnering with physicians to optimize every part of your drug therapy.

Patients managing multiple conditions are at the highest risk of medication-related problems, and our pharmacist specializes in complex cases with many medications. There is no separate referral and no extra appointment required.

Pharmacist services

Medication therapy management

Comprehensive reviews to identify inappropriate medications, unnecessary duplications, dosing errors, and drug interactions, with an action plan for your prescribing physician.

Adherence coaching

Personalized strategies to help you take medications correctly and consistently, addressing the cost, confusion, side effects, and scheduling barriers that lead to missed doses.

Drug interaction screening

A systematic review of all medications, including supplements and over-the-counter products, to detect significant interactions before they cause harm. This matters most for patients on five or more medications.

Patient and caregiver education

Plain-language counseling on what each medication does, when and how to take it, which side effects to watch for, and which symptoms warrant a call to the care team.

Cost and formulary optimization

Working with physicians to identify therapeutically equivalent, lower-cost alternatives, so you can afford and access your medications without sacrificing effectiveness.

Transitions of care reconciliation

At discharge from a hospital or skilled nursing facility, the pharmacist reconciles medications to catch discrepancies, discontinued drugs, and new prescriptions, helping prevent readmissions from medication errors.

Connected Care

Support across our programs.

Chronic care management

Patients in chronic care management receive routine pharmacist reviews as part of their monthly care plan.

Remote patient monitoring

Readings from home devices, such as blood pressure, help inform the pharmacist’s recommendations on related medications.

Getting Started

The pharmacist review process.

1

Medication list collection

All medications, including prescriptions, over-the-counter products, vitamins, and supplements, are compiled into one complete list during your visit or as part of chronic care management enrollment.

2

Clinical review

Our pharmacist reviews the list against your diagnoses, labs, and vitals to find interactions, inappropriate medications, and dosing issues.

3

Physician collaboration

Findings and recommendations go directly to your physician, who reviews them and decides on any changes.

4

Patient education

You and your caregiver receive plain-language counseling on your updated regimen: what changed, why, and what to watch for.

5

Ongoing monitoring

For patients in chronic care management or remote monitoring, the pharmacist takes part in ongoing monitoring and adjusts recommendations as conditions and medications change.
